Service Description
Our DOT 5-Panel Urine Drug Test is performed in full compliance with U.S. Department of Transportation (DOT) regulations for employees in safety-sensitive positions. Unity Diagnostics provides accurate, federally compliant testing for employers, owner-operators, and DOT-regulated organizations while maintaining strict chain-of-custody procedures and laboratory standards. The DOT 5-panel screens for: ● Marijuana (THC) ● Cocaine ● Amphetamines (including methamphetamine) ● Opiates ● Phencyclidine (PCP) We offer all federally required DOT testing services, including: ● Pre-employment ● Random ● Post-accident ● Reasonable suspicion ● Return-to-duty ● Follow-up All specimens are collected by trained collectors, analyzed by a certified laboratory, and reviewed by a Medical Review Officer (MRO). DOT testing is available for commercial drivers, aviation, railroad, pipeline, maritime, and other DOT-regulated industries. Additional Collection Fees ● Shy Bladder Collection: $50 ● Direct Observation or Temperature Out of Range Collection: $50 ● After-Hours Collection: $75
Cancellation Policy
A cancellation will not result in a refund for any reason. However, you may reschedule your appointment as long as it is at least 48 hours before your appointment. If it is not at least 48 hours before your appointment, you will be charged a $50.00 reschedule fee. No call, no show will result in you having to pay for another test.
